listed for each individual physician are intended to be indicative of the health
plans the physician participates in through NTSP. The physician you select may not
participate in all plans listed, or may participate in additional health plans which
may not be posted due to individual office changes or updates that have not yet
reflected on the website. Please verify the physician's participation in a particular
plan by contacting the physician's office directly.

The reports and information on the HMO tab of the quality rankings website are reprinted
with the permission of the Texas Health Care Information Council (THCIC) and contains
data from reports provided to NTSP by the THCIC. NTSP assumes no responsibility
for any errors. The full report for HMO quality can be found on the THCIC website
at www.thcic.state.tx.us.

Physician Site
As a user ("You" or "Your") of the www.myntsp.com website (the "Physician Access
Program") You agree that by logging in with the username and password assigned to
You and/or Your use of the Physician Access Program, You agree to all website policies
and the participation agreement in place between the Physician practice to which
You are assigned (the "Physician") and NTSP. You agree that any use of tools or
functions contained within the Physician Access Program will be for the sole use
of conducting business for the Physician. You agree that You will not release any
portion of the Physician Access Program, including but not limited to, data contained
on the site and screen prints of any page of the site, to any third party without
the express written consent of NTSP.
Confidentiality of Website
Any tools You use in the Physician Access Program should be used for the purpose
of conducting business for the Physician. You understand that all contract details,
including but not limited to, contract provisions, comparison tools, and reimbursement
rates are confidential and should not be released to any party, including but not
limited to any other physician in Your same Physician practice, without the express
written consent of NTSP.
Patient Privacy
You understand that access to or use of any function containing member information,
including the Referral/Pre-Certification form or Patient Profile function is covered
under the Business Associate Agreement between the Physician practice and NTSP.
You agree to protect patient information You access from this website pursuant to
the guidelines outlined by HIPAAA and any other current federal and/or state regulations
outlining patient privacy. You understand that a violation of this policy may result
in immediate denial of Your access to the Physician Access Program.
